Pennsylvania Family Caregiver Program
Application Checklist
Everything you need to apply for Medicaid-funded caregiver pay in PA — documents, eligibility criteria, and step-by-step instructions in one printable checklist.
Pennsylvania offers several programs that pay family members to care for a loved one at home. This checklist covers the two primary Medicaid-funded pathways: Community HealthChoices (CHC) and the OPTIONS Program through Philadelphia Corporation for Aging. A-Team Home Care is an enrolled provider in both.
Part 1 — Am I Eligible?
Check all that apply to your loved one (the person who needs care):
If you checked 4 or more boxes: your family member very likely qualifies. Call A-Team at (215) 490-9994 for a free eligibility review.
Part 2 — Documents to Gather
Collect these before starting the application. Missing documents are the #1 reason applications are delayed.

Part 3 — Step-by-Step Application Process
Call A-Team for a free eligibility screening
We verify Medicaid enrollment and care level in about 10 minutes. Call (215) 490-9994 or submit the form on this page.
Care coordinator visits the home
An A-Team nurse or coordinator assesses the care recipient’s needs and creates a written care plan. This is required by Medicaid before services can start.
You complete A-Team’s caregiver onboarding
Background check, TB test, CPR/First Aid training, and a brief orientation. A-Team coordinates everything — typically completed within 1–2 weeks.
Authorization approved by the Managed Care Organization
The care plan is submitted to the CHC MCO (Keystone First, UPMC CHC, or PA Health & Wellness). Approval typically takes 2–4 weeks. A-Team tracks the status on your behalf.
You start getting paid — bi-weekly direct deposit
Once authorized, you clock in/out using A-Team’s mobile app. Pay is deposited every other Friday directly to your bank account.
Part 4 — Common Questions
Can a spouse be the paid caregiver?
Yes. Under Community HealthChoices in Pennsylvania, a spouse can be employed as the paid caregiver. This is a major change from older rules. The couple must live together and the care recipient must be enrolled in CHC Medicaid.
How many hours per week can I work as a caregiver?
Hours are determined by the care plan and MCO authorization — typically 20–56 hours per week depending on the level of care need. A-Team’s care coordinator will document the appropriate hours during the home assessment.
What is the hourly pay rate?
Call A-Team at (215) 490-9994 for current rates in your county. Rates vary based on service type and are set by the MCO. A-Team will give you the exact figure for your situation during the free screening call.
What if my family member isn’t on Medicaid yet?
A-Team can help you start the Medicaid application. Many seniors and people with disabilities who need care qualify but have never applied. Call us and we will walk you through what’s needed.
