How 24-Hour Home Care Works at A-Team

A-Team Home Care staffs 24-hour cases with rotating shift caregivers — not a single live-in caregiver who may be asleep. We use 8- or 12-hour shifts depending on the case, with shift overlaps to ensure a smooth handoff and continuous awareness of the client’s condition. Each caregiver receives a care plan and shift notes from the previous caregiver.

We have established relationships with discharge planners at Temple University Hospital, Jefferson Health, Penn Medicine, CHOP, and Aria Health across Philadelphia and Bucks County.
