WHO SKILLED NURSING IS FOR

4 Clinical Situations That Most Often Lead to Skilled Nursing

Most A-Team skilled nursing patients come home in one of these four clinical situations.

Post-surgical wound care

Discharged home but not fully healed after a joint replacement, cardiac, or abdominal surgery. An RN manages the incision and watches for infection.

Diabetic management at home

Newly insulin-dependent, uncontrolled A1C, or diabetic wounds. Skilled nursing brings monitoring, teaching, and wound care to the home.

Stroke rehabilitation

Returning home from the hospital or inpatient rehab. A-Team’s coordinated PT, OT, and speech therapy continue the recovery at home.

Hospice transition support

When a patient is moving toward comfort-focused care, skilled nursing at home provides clinical comfort care and family support.

THE HOME ADVANTAGE

Clinical Care With the Human Element Intact

The difference between home-based skilled nursing and institutional care is not just location. At home, the RN visit is one-on-one with the patient and family, no ward rounds, no shared rooms, no strangers in the next bed. Family members learn what to watch for. The physician gets direct reports from the nurse who has actually been in the home. A-Team RNs are not moving patient to patient down a hospital hallway; they are in your loved one’s home, with their complete history and full attention.

ONE AGENCY, TWO LEVELS OF CARE

Skilled Nursing + Personal Care, Same Agency, No Gaps

Many skilled nursing patients also need daily personal care, bathing, dressing, transfers, and meals, between RN visits. A-Team provides both under the same plan of care. The RN supervises the home-health aide so the hands-on care stays aligned with the clinical picture. No handoffs, no duplicate intakes, and where possible the same aide before and after the Medicare episode ends.

Skilled Nursing FAQ

The most common questions Greater Philadelphia and Bucks County patients and families ask about Medicare-certified skilled nursing at home.

What is skilled nursing at home?
Skilled nursing at home is clinical care delivered in the patient’s residence by a registered nurse under a physician order. It includes wound care, vital-sign monitoring, medication management, disease management, and patient education. It requires a licensed nurse and cannot be delegated to an aide or companion.
Does Medicare cover skilled nursing at home?
Yes. Medicare Part A covers home-health skilled nursing when three conditions are met: the patient is homebound, there is a documented skilled need, and there is a physician order. A-Team is Medicare-certified, and coverage is at 100% with no copay for qualifying visits.
What is the difference between skilled nursing and personal care?
Skilled nursing is clinical care requiring a licensed RN or therapist, such as wound care, therapy, and disease management. Personal care is non-clinical help with bathing, dressing, and mobility. A-Team provides both and coordinates them under one plan.
How quickly can skilled nursing start after discharge?
When the hospital faxes the discharge order with the physician’s signed plan of care, A-Team confirms eligibility the same day and schedules the start-of-care RN visit within 48 hours of discharge.
Do you provide physical, occupational, and speech therapy?
Yes. A-Team provides all three therapies in-house under the same home-health episode, coordinated with the nursing plan of care.
Can the same agency provide care after the skilled episode ends?
Yes. When skilled needs resolve, A-Team transitions the client to personal or companion care under the same agency, with no new intake and, where possible, the same aide.
