WHO CAN APPLY
Who Qualifies to Get Paid
A short call confirms eligibility for the care recipient and the caregiver.
The care recipient
Your loved one is enrolled in or eligible for Pennsylvania Medicaid through Community HealthChoices, or is 60 or older through OPTIONS.
A real care need
They need help with daily activities such as bathing, dressing, meals, or medication reminders.
The family caregiver
An adult family member or trusted person who provides the care. A-Team hires you on as the paid caregiver.
A-Team is in-network with all three Community HealthChoices plans and handles the hiring and payroll.

Documents You Will Need
- ✓Photo ID for the caregiver and the care recipient
- ✓The care recipient Medicaid or Community HealthChoices ID, or proof of eligibility
- ✓Proof of relationship or residence
- ✓A basic list of the daily tasks you help with
- ✓Bank details for caregiver payroll setup

How to Apply and Get Paid
A-Team handles enrollment, hiring, and payroll for you.
1. Free eligibility call
We confirm your loved one program eligibility and answer your questions on one short call.
2. Enroll in the program
We help enroll your loved one in Community HealthChoices or OPTIONS and select self-directed care.
3. A-Team hires you
We bring you on as the paid family caregiver, handle the paperwork, and set up payroll.
4. Get paid to care
You provide the care you already give, and you are paid for it on a regular schedule.

| PA Medicaid (CHC) | Covers personal-care hours that can reach near-24-hour coverage for the highest assessed need; A-Team is in-network with all three plans. |
| Veterans Benefits | VA Aid & Attendance for eligible veterans and surviving spouses. |
| Long-Term Care Insurance | Many LTC policies reimburse 24-hour in-home care. |
