WHO CAN APPLY
A short call confirms eligibility for the care recipient and the caregiver.
Your loved one is enrolled in or eligible for Pennsylvania Medicaid through Community HealthChoices, or is 60 or older through OPTIONS. Eligibility and enrollment rules come from the Pennsylvania Department of Human Services (Community HealthChoices).
They need help with daily activities such as bathing, dressing, meals, or medication reminders.
An adult family member or trusted person who provides the care. A-Team hires you on as the paid caregiver.
A-Team is in-network with all three Community HealthChoices plans and handles the hiring and payroll.
